T3 Uptake Thyroid Test

Thyroid Binding Test Used with T4 to Help Calculate Free Thyroxine Index

The T3 Uptake Thyroid Test is a blood test used to help evaluate thyroid hormone binding in the blood. It is commonly used with a thyroxine test, also called T4, to help calculate the Free Thyroxine Index, or FTI.

Important: T3 uptake does not measure the thyroid hormone T3. Despite the name, this test is an indirect measure of thyroid hormone binding capacity and should not be used alone.

This service provides access to lab testing only. It is intended as a screening and monitoring tool and does not diagnose, treat, or replace care from a licensed healthcare provider.

What This Test Measures

T3 Uptake

T3 uptake reflects available thyroid hormone binding sites in the blood. It is often used with Total T4 to help estimate free thyroid hormone activity through the Free Thyroxine Index.

  • Test name: T3 Uptake
  • Also known as: T3RU, T3 Resin Uptake, Thyroid Hormone Binding Ratio, THBR
  • Sample type: Blood
  • Purpose: Thyroid binding assessment
  • Important note: This is not a serum T3 hormone test

T3 Uptake Is Not T3

The name can be confusing. T3 uptake does not measure how much T3 thyroid hormone is in your blood.

If your provider wants to measure the actual T3 hormone, they may order Total T3 or Free T3 instead. If your provider wants to evaluate binding proteins or calculate Free Thyroxine Index, T3 uptake may be appropriate.

Why Order a T3 Uptake Test?

  • Thyroid panel support - T3 uptake may be used with T4 to help calculate Free Thyroxine Index.
  • Binding protein evaluation - May help when thyroid hormone binding proteins may affect interpretation.
  • Provider-directed testing - Useful when a healthcare provider specifically requests T3 uptake.
  • Thyroid function review - May support broader thyroid testing when interpreted with TSH, T4, or other thyroid markers.

Who May Consider This Test?

This test may be appropriate for:

  • Individuals whose healthcare provider requested T3 uptake testing
  • People completing a thyroid panel that includes T3 uptake
  • Patients whose provider wants Free Thyroxine Index calculation
  • Individuals being evaluated for thyroid hormone binding changes
  • Customers who need doctor-ordered thyroid testing without using insurance

When T3 Uptake May Be Helpful

T3 uptake may be helpful when thyroid hormone binding proteins are affecting thyroid test interpretation. Binding proteins can change in certain situations, such as pregnancy, estrogen use, oral contraceptive use, some liver or kidney conditions, and certain medication use.

Your healthcare provider can determine whether T3 uptake, Free T4, Total T4, TSH, or another thyroid test is most appropriate.

Important Biotin Warning

If you take biotin, also known as vitamin B7, vitamin B8, vitamin H, or coenzyme R, ask your healthcare provider whether you should stop biotin before testing.

Biotin may interfere with certain thyroid lab testing methods and could affect results.

Important: Screening Only

LabReqs and SchoolTiters provide access to lab testing. We do not diagnose hypothyroidism, hyperthyroidism, thyroid disease, pituitary disease, autoimmune thyroid disease, or any medical condition.

Results should be reviewed with a licensed healthcare provider, especially if symptoms are present, results are abnormal, or you take thyroid medication.

Important Limitations

  • T3 uptake should not be interpreted by itself.
  • This test does not measure serum T3 or Free T3.
  • This test does not diagnose thyroid disease by itself.
  • Results are usually most meaningful when reviewed with T4, TSH, Free T4, or other thyroid markers.
  • This test does not include TSH, Total T4, Free T4, Free T3, thyroid antibodies, or reverse T3 unless ordered separately.

Before Ordering

  • Confirm that your provider requested T3 uptake, not Total T3 or Free T3.
  • If your provider requested a full thyroid panel, confirm whether additional thyroid tests are needed.
  • Do not stop thyroid medication unless your healthcare provider tells you to.
  • If you take biotin, ask your provider whether to stop it before testing.
  • If you are unsure which thyroid test you need, ask your healthcare provider before ordering.

Frequently Asked Questions

Does T3 uptake measure T3?

No. T3 uptake does not measure the T3 hormone. It measures thyroid hormone binding capacity.

What is T3 uptake used for?

T3 uptake is often used with Total T4 to help calculate the Free Thyroxine Index and support thyroid function interpretation.

Should T3 uptake be ordered by itself?

Usually no. T3 uptake is typically interpreted with other thyroid tests, especially T4 and TSH.

Is this the same as Free T3?

No. Free T3 directly measures unbound T3 hormone. T3 uptake is a different test.

Do I need to fast?

Fasting is usually not required unless your healthcare provider gives different instructions.

Does this test diagnose thyroid disease?

No. This test provides lab information only. Diagnosis and treatment decisions must be made by a licensed healthcare provider.

Is a doctor's order included?

Yes. A doctor's order is included with this lab test.
