Florida immunization rules for students

College and university students

Florida law addresses meningococcal meningitis and hepatitis B vaccination information at postsecondary institutions; each college and clinical program sets its own documentation requirements. (source)

Students ages 18–23 vaccinated at a college or university student health center may opt out of inclusion in Florida SHOTS. (source)

K–12 school entry (including high-school CNA and dual-enrollment students)

Florida K–12 students generally need 4–5 DTaP, 4–5 IPV, 2 MMR, 3 hepatitis B, 1 Tdap, and 2 varicella doses. Seventh graders additionally need Tdap. Provider-documented varicella disease may satisfy the varicella requirement. (source)

Many programs accept a quantitative titer as proof of immunity when vaccine records are missing or incomplete. If your program requires a specific test, such as a quantitative Hep B surface antibody or a QuantiFERON TB blood test, order that exact test.

Florida's immunization registry is Florida SHOTS (State Health Online Tracking System). Ask parents or caregivers, schools, previous employers, or the local county health department for records. Individuals may also contact a primary care provider or local health department; Florida advises calling 877-888-7468 for Florida SHOTS information. In Alachua County, photo ID is required.
