College and university students

K–12 school entry (including high-school CNA and dual-enrollment students)

Florida K–12 students generally need 4–5 DTaP, 4–5 IPV, 2 MMR, 3 hepatitis B, 1 Tdap, and 2 varicella doses. Seventh graders additionally need Tdap. Provider-documented varicella disease may satisfy the varicella requirement. (source)

Many programs accept a quantitative titer as proof of immunity when vaccine records are missing or incomplete. If your program requires a specific test, such as a quantitative Hep B surface antibody or a QuantiFERON TB blood test, order that exact test.

Florida's immunization registry is Florida SHOTS (State Health Online Tracking System). Ask parents or caregivers, schools, previous employers, or the local county health department for records.
